  • Initial Stage: Hormonal Tests and Ovarian Ultrasound/ Eligibility for Using Own Eggs in an IVF Cycle. Before undergoing intensive medical checkups, it is recommended to conduct hormonal checks to assess the potential for stimulation outcomes. Begin with an ovarian ultrasound to ascertain the follicle count in each ovary and understand the location of the ovaries, gaining insights into their accessibility for vaginal retrieval. It is important to note that for a detailed assessment, we mandatorily require ovarian ultrasound scan pictures and a comprehensive report. The report should include measurements of all follicles, including those of smaller sizes, presented in millimeters in a detailed manner. The following blood tests are essential:
    • AMH (Anti-Mullerian Hormone) (can be done on any day of the menstrual cycle, with a validity term of 6 months)
    • LH (Luteinizing hormone) – (performed on day 2-3 of the menstrual cycle, with a validity term of 3 months).
    • FSH (Follicle-Stimulating Hormone) – (performed on day 2-3 of the menstrual cycle, with a validity term of 3 months).

Once the results of the hormonal tests and ovarian ultrasound are available, our medical team reviews them thoroughly. Upon confirmation that the results are satisfactory and meet the required criteria, you receive approval to proceed to the second stage of the checkup process. 

  • Second Stage: All Remaining Checkups. After obtaining approval from our medical team, the remaining checkups can be scheduled at any day of the menstrual cycle. This flexibility allows for personalized planning and accommodates individual preferences. 

By implementing this staged approach, we aim to ensure a more manageable and efficient process for individuals considering IVF with their own eggs and sperm. It is important to note that medical evaluations are often adjusted to individual needs and results may vary. If any of mentioned medical checkup results reveals values that indicate the necessity for further related assessments to provide a more accurate evaluation, the medical team may recommend additional checkups. The purpose of these additional examinations is to gather more information and ensure a comprehensive understanding of the individual’s health status.

Stimulation details 

In our stimulation process, we implement short antagonist protocols, typically spanning 12-14 days. Here are more details:

  1. Short Antagonist Protocols (12-14 days): This refers to a specific approach in ovarian stimulation where GnRH antagonists are used to prevent premature ovulation. The protocol is called “short” because it typically involves a shorter duration compared to long protocols.
  2. Stimulation Medications:
  • Gonal F: This is a follicle-stimulating hormone (FSH) used to stimulate the growth of ovarian follicles.
  • Menopur: It contains a combination of FSH and luteinizing hormone (LH), also used for ovarian stimulation. 
  • Cetrotide: This is a GnRH antagonist that helps prevent premature ovulation by blocking the action of GnRH.
  • Ovitrelle / Choragon: This is human chorionic gonadotropin (hCG), which is used as the final trigger to induce the final maturation and release of eggs from the follicles.

The careful structuring of medication doses based on complete medical checkup evaluation and on factors such as age, BMI, and other medical conditions is a personalized approach to optimize the chances of success while minimizing risks. The goal is to stimulate the ovaries to produce multiple mature eggs for retrieval during the IVF process and avoid hyperstimulation or any stimulation related medical complications.

Two options of Stimulation

In our commitment to ensuring the utmost comfort for intended parents on their path to parenthood, we provide two distinct options for the stimulation process. You may choose to undergo the complete stimulation journey right here in Georgia, or alternatively, initiate the process at your local clinic and join us on day 9 of stimulation.
  • Option 1: Complete Stimulation in Georgia

For those opting to complete the stimulation in Georgia, we have a mandatory requirement. Intended parents are requested to perform an ovarian ultrasound 4-5 days before the anticipated period when stimulation is planned. Our experienced IVF specialists meticulously review the ultrasound images to confirm the absence of cysts or lingering follicles from previous ovulations. This precautionary step is essential to prevent any delays or cancelation of stimulation after your arrival.

Upon receiving confirmation from our specialists, we request intended parents to be available 1-2 days prior to the expected onset of menstruation. Once the menstrual cycle begins, a visit to our clinic is organized, and the stimulation process commences.

For individuals with unique circumstances, such as those with Asherman syndrome, MRKH syndrome, or those who have undergone a hysterectomy, we implement a customized approach. A 14-day course of contraceptive pills, initiated at any random day of the month, prepares the body for the upcoming stimulation. An ovarian ultrasound is performed on the last day of contraceptive pill use, and the intended parents are required to be present at our clinic four days after completing the pills, marking the commencement of the stimulation process.

Throughout the stimulation phase, mandatory visits on days 1, 5, and 8 ensure close monitoring of the progress. Subsequent visits are planned based on follicle sizes and individual responses to hormonal injections. Intended parents should allocate approximately 17 days for the complete stimulation process, including the recovery period.

  • Option 2: Initiate Stimulation Locally and Join Us on Day 9

For intended parents considering the option to commence stimulation at their local clinic and subsequently join our clinic from day 9 of the process, we understand the challenges faced with some international IVF clinics. Some clinics may prefer to oversee the entire IVF procedure, leading to frustration for those seeking assistance solely during the initial stimulation phase.

If you have identified a local IVF clinic willing to collaborate with us in supporting your stimulation, we are eager to participate and assist in structuring the stimulation protocol for you. To facilitate this collaboration, we request the following considerations:

  • Pre-Stimulation Requirements:
    Day 1 of Menstruation: Perform LH, FSH, Estradiol, and ovarian ultrasound scans. Share the results with our IVF team.
    Stimulation Protocol: Based on the provided results, our IVF team will customize the stimulation protocol, including doses and prescriptions for stimulation injections.
  • Monitoring and Adjustment:
    Day 5 and 8 of Stimulation: Conduct Estradiol and ovarian ultrasounds locally. Share the results with our IVF team for ongoing monitoring and adjustment of the stimulation protocol. Cetrotide injection will be added at this stage and precise timing if crucial when adding cetrotide injection.
  • On-site Support:
    Day 9 of Stimulation: It is mandatory for the intended parent to be present at our clinic in Georgia on day 9. Our experienced IVF doctor will take over the ultrasounds and assume full responsibility for the remainder of the stimulation process.

Flexible Options for Seamless IVF Journey

For intended parents who face challenges in performing medical checkups in their home countries and prefer a comprehensive approach, we offer flexible solutions to ensure a smooth and successful IVF journey. When you are prepared to undergo stimulation, we invite you to travel to Georgia 5-6 days before the expected onset of menstruation. It is crucial to perform an ovarian ultrasound before traveling to rule out the presence of any cysts. Upon your arrival, our experienced team will conduct all necessary medical checkups, ensuring a thorough review of every aspect of your health before proceeding with the stimulation process. If all medical checkup results are satisfactory, the stimulation process will commence on day 2 of your menstrual cycle. This approach allows for a streamlined and closely monitored IVF journey, with all necessary assessments conducted in-house. However, please be aware that there is a slight risk associated with this plan. In some cases, unforeseen medical conditions may be identified during the initial checkups, which may pose a temporary hindrance to starting stimulation on the current menstrual cycle. In such instances, additional discussions and potential adjustments to the plan may be needed, and you may be required to return for further assessment after the initial visit. 

Important Notes:

  • When administering hormonal injections, follow the specified timing provided by your doctors. The injection schedule is clearly outlined in your prescriptions, emphasizing the importance of injecting at the same time daily. Consistency in timing is key to ensuring the desired response to the stimulation.
  • For medical checkups referencing specific days of the menstrual cycle, strict adherence is crucial. This ensures accurate assessments and contributes to the overall success of the IVF process. For intended parents without regular menstrual cycles, all medical checkups are conducted on random days of the month. Our doctors are well-informed about this when reviewing hormonal screenings.
  • Punctuality is essential when arriving at the clinic for scheduled appointments. Morning blood tests are conducted during these appointments, allowing our doctors adequate time to receive and analyze the results. This timely process is integral to adjusting protocols accordingly and ensuring the effectiveness of your personalized IVF journey. Your cooperation in following these guidelines will contribute to a seamless experience, and should you have any questions or concerns, our team is readily available to assist you.
  • If you are prescribed contraceptive pills, it is crucial to take them daily at the same time without missing a day. For instance, if you start the pill at 6 pm, maintaining this schedule is essential for optimal effectiveness. Deviating from this routine may compromise the contraceptive pill’s efficacy, and in such cases, prompt communication with your program manager is strongly advised.

There are various types of contraceptive pills designed to serve specific purposes for patients. During the initial phase for starting stimulation, it is crucial to use contraceptive pills with a special combination of estrogen and progesterone. Our doctors typically recommend pills such as Yasmin, Yarina, Regulon, and Regividon, which come in a 21-pill pack. If your local doctor prescribes a different contraceptive pill within the same class, it is important to discuss this with your program manager and ensure you have approval from us before starting the new pill.
